Aligning Forces for Quality/Memphis

The Americans for Quality Health Care Team works with the Aligning Forces for Quality (AF4Q) Alliances.

AF4Q is the Robert Wood Johnson Foundation’s (RWJF) signature effort to lift the overall quality of health care in targeted communities, reduce racial and ethnic disparities and provide models for national reform.

AF4Q asks the people who get care, give care and pay for care to work together toward common, fundamental objectives to lead to better care.

Healthy Memphis Common Table (the Alliance) leads the area's AF4Q initiative. For more information on the Alliance, visit their community snapshot or their web site.

Alliance of Community Health Plans
The Alliance of Community Health Plans is a network of health plans and provider organizations whose members collaborate to identify problems, share information and work toward health care solutions. Health care quality news, including such topics as measurement and public reporting, are available by clicking on the “Public Policy” button at the top of the homepage.

Be MedWise Tennessee
Be MedWise Tennessee is an educational program designed to improve the medication literacy of Tennesseans in partnership of the National Council on Patient Information and Safety, the University of Tennessee College of Pharmacy and the University of Tennessee Extension. Be MedWise Tennessee provides quality and accurate information on making safe and appropriate decisions using over-the-counter (OTC) and prescription drugs.

HealthCare 21 Business Coalition
HealthCare 21 Business Coalition is a nonprofit, member driven organization committed to improving the quality and cost of health care in Knoxville, Chattanooga, Cleveland and Nashville regions.

Healthy Memphis Common Table (HMCT)
HMCT is a regional health and health care improvement collaborative for the greater Memphis area.

The Leapfrog Group
The Leapfrog Group is a national public-private partnership that works to reduce medical errors and improve patient safety. The Leapfrog Group provides the latest hospital patient safety performance results in Tennessee.

Memphis Business Group on Health
The Memphis Business Group on Health (MBGH) facilitates the purchase of efficient and effective health care services for the Memphis community. MBGH has 37 member and affiliates that employ approximately 50,000 employees in the greater Memphis area.

MidSouth eHealth Alliance
Health care providers in the Memphis area share information on care provision through the MidSouth eHealth Alliance, a community wide health information system.

QSource
QSource is a not-for-profit health care consulting firm and the Medicare Quality Improvement Organization (QIO) for Tennessee working to refine delivery systems so patients get the right care at the right time. Medicare beneficiaries and their families have access to information on physicians, hospitals, nursing homes, home health agencies and other caregivers.

Society for Healthcare Consumer Advocacy, Tennessee Chapter (SHCA, Tennessee Chapter)
SHCA, Tennessee Chapter is a membership organization advancing health care consumer advocacy by supporting the role of professionals who represent and advocate for consumers across the health care continuum. Consumers can access information on patient education, patient’s rights, ethics and patient satisfaction measurement.

Tennessee Association for Healthcare Quality (TAHQ)
TAHQ is a membership organization of health care professionals advancing health care quality and patient safety through the development and support of initiatives designed to enhance the effectiveness of health care quality professionals. TAHQ members specialize in quality/performance improvement, patient safety, case management, utilization review, infection control, and risk management.

Tennessee Department of Health
The Tennessee Department of Health is the state’s lead public health agency. It conducts programs to protect and improve the health of entire communities, including programs that promote clean water, safe food, quality health care, and healthy personal choices. Reports on patient safety practices, preventing medical errors, nursing home compare surveys and consumer health care information, including the “Tennessee Improving Patient Safety (TIPS)” Report are available.

Tennessee Health Care Choices
This website, hosted by BlueCross BlueShield of Tennessee, provides information on health care choices, costs and value in Tennessee.

Tennessee Hospital Association (THA)
THA is a not-for-profit membership association that serves as an advocate for Tennessee hospitals, health systems and other health care organizations and the patients they serve. Consumers can find health links and resources including the 2006 Healthcare Acronym Guide.

Tennessee Hospital Quality Comparison
Tennessee Hospital Quality Comparison allows consumers to compare facilities in the state to make better informed health care decisions.
