Aligning Forces for Quality/Cincinnati
The Americans for Quality Health Care Team works with the Aligning Forces for Quality (AF4Q) Alliances.
AF4Q is the Robert Wood Johnson Foundation’s (RWJF) signature effort to lift the overall quality of health care in targeted communities, reduce racial and ethnic disparities and provide models for national reform.
AF4Q asks the people who get care, give care and pay for care to work together toward common, fundamental objectives to lead to better care.

A Consumer Guide to Ohio Hospital Quality
This report, a publication of the Employers Health Coalition of Ohio, compares hospitals in the state of Ohio against the average performance of all hospitals on three key indicators of quality: mortality rates, complication rates, and the rate at which hospitals are able to prevent the occurrence of a complication from evolving into a death (also called failure to rescue). In addition to these outcome measures, the Guide also includes process of care measures.
Greater Cincinnati Health Council
Greater Cincinnati hospitals voluntarily report on 4 measures for 2007:
- Heart attack process of care measures;
- Congestive heart failure process of care measures;
- Pneumonia process of care measures; and
- Risk-adjusted mortality and length of stay measures.
The Leapfrog Group
The Leapfrog Group is a national public-private partnership that works to reduce medical errors and improve patient safety. The Leapfrog Group provides the latest hospital patient safety performance results in Cincinnati for the Christ Hospital and Cincinnati Children’s Hospital Medical Center.
National Committee for Quality Assurance
A voluntary program designed to recognize physicians who use evidence-based measures and provide excellent care to their patients in the areas of heart/stroke care, diabetes care, back pain and systematic processes. Cincinnati has physicians in all categories.

This website has various statistics about health care disparities in Ohio.
Ohio Department of Health Hospital Performance Measures Data
The Ohio Department of Health is the state’s lead public health agency. The Department’s website contains hospital performance measures data on nine specific services which are generally provided by hospitals. Ohio’s requirement is not an error reporting requirement.
Ohio KePro
Ohio’s Quality Improvement Organization works to protect Medicare patients rights, runs a Medicare beneficiary helpline, receives quality of care complaints, runs a discharge dispute program and works with patients on dispute resolution with their Medicare provider.
Ohio Patient Safety Institute (OPSI)
OPSI is an organization dedicated to improving patient safety in Ohio. The Institute is a subsidiary of the Ohio Health Council, which was founded by the Ohio Hospital Association, the Ohio State Medical Association, and the Ohio Osteopathic Association. This website has information on medication safety, how to prevent hospital infections and other safety issues.
State of Ohio Long-Term Care Consumer Guide
This website provides information about nursing homes and assisted living facilities to help consumers, family members and professionals search for the appropriate facility to meet an individual’s needs.
