The Americans for Quality Health Care Team works with the Aligning Forces for Quality (AF4Q) Alliances.

AF4Q is the Robert Wood Johnson Foundation’s (RWJF) signature effort to lift the overall quality of health care in targeted communities, reduce racial and ethnic disparities and provide models for national reform.

AF4Q asks the people who get care, give care and pay for care to work together toward common, fundamental objectives to lead to better care.

MN Community Measurement (the Alliance) leads the area's AF4Q initiative. For more information on the Alliance, visit their community snapshot or their web site.

Citizens’ Council on Health Care
The Citizens’ Council on Health Care is a non-profit organization working to enable individuals to control health care decision making. They provide information on consumer-driven health care and evidence-based medicine.

The Leapfrog Group
The Leapfrog Group is a national public-private partnership that works to reduce medical errors and improve patient safety. The Leapfrog Group provides the latest hospital patient safety performance results in Minnesota.

Minnesota Alliance for Patient Safety
The Minnesota Alliance for Patient Safety is a partnership of more than 50 public-private health care organizations working together to improve patient safety. Consumers will find information and tools to assist in tracking reports on their medicines, including prescription and over-the-counter drugs.

Minnesota Community Measurement Health Care Quality Report
This report provides consumers with information on how provider groups perform on preventive care screenings such as breast cancer screenings, and how they provide care for asthma, diabetes, and other chronic diseases.

Minnesota Department of Health
The Minnesota Department of Health is the state’s lead public health agency. The Department’s website contains the annual Adverse Health Events report as well as links to reports and organizations related to patient safety.

Minnesota Health Information
This web site provides a wide range of information about the cost and quality of health care in Minnesota, including the ability to compare certain health care providers, learn how to be a savvy health care consumer, or better manage personal health.

Minnesota Hospital Association
Minnesota Hospital Association is a trade association representing Minnesota's hospitals and health systems. The web site provides health links and resources, including information on how to make the health care system work for you.

Minnesota Medical Association—Quality Organizations
A list of local and national organizations dedicated to improving medical care.

Multicultural Health Care Resources
These links offer health care resources for health care consumers and providers as well as information on eliminating health disparities.

Stratis Health
Stratis Health is a non-profit independent quality improvement organization that collaborates with providers and consumers to improve health care. Consumers can find information about nursing homes, home health care, and public reporting of health information.
