Aligning Forces for Quality/West Michigan

The Americans for Quality Health Care Team works with the Aligning Forces for Quality (AF4Q) Alliances.

AF4Q is the Robert Wood Johnson Foundation’s (RWJF) signature effort to lift the overall quality of health care in targeted communities, reduce racial and ethnic disparities and provide models for national reform.

AF4Q asks the people who get care, give care and pay for care to work together toward common, fundamental objectives to lead to better care.

Aligning Forces for Quality in West Michigan (the Alliance) leads the area's AF4Q initiative. For more information on the Alliance, visit their community snapshot or their web site.

The participation of consumer organizations in health care quality initiatives is critical to effect patient-centered changes in our health care system.

Alliance for Health
The Alliance for Health is a nonprofit organization in Western Michigan that assists with resources planning, health education, and health delivery systems information sharing.

The Leapfrog Group
The Leapfrog Group is a national public-private partnership that works to reduce medical errors and improve patient safety. The Leapfrog Group provides the latest hospital patient safety performance results in Michigan.

Michigan Health & Hospital Association - Keystone Center for Patient Safety & Quality
The Keystone Center for Patient Safety & Quality is non profit division of the Michigan Health & Hospital Association (MHA) Health Foundation. MHA Keystone brings together hospitals, national experts and best practice evidence to improve patient safety by addressing the quality of health care delivery at the bedside.

Michigan Primary Care Consortium
The Michigan Primary Care Consortium is a collaborative partnership created to improve the system of delivery of prevention services and the management of chronic disease and other conditions in primary care settings throughout Michigan by aligning quality improvement initiatives, addressing gaps, and engaging in problem-solving strategies to ensure a patient-centered medical home for everyone.

Michigan Quality Improvement Consortium
The Michigan Quality Improvement Consortium or MQIC is a collaborative of physicians and personnel representing the Michigan HMOs along with the Michigan State Medical Society, the Michigan Osteopathic Association, the Michigan Association of Health Plans, the Michigan Peer Review Organization and Blue Cross Blue Shield of Michigan. The MQIC establishes and implements a core set of clinical practice guidelines and performance measures.

Michigan Quality Improvement Organization
Michigan Quality Improvement Organization is a non-profit, independent quality improvement organization that collaborates with Medicare beneficiaries to improve their health care.
