Aligning Forces for Quality/Maine

The Americans for Quality Health Care Team works with the Aligning Forces for Quality (AF4Q) Alliances.

AF4Q is the Robert Wood Johnson Foundation’s (RWJF) signature effort to lift the overall quality of health care in targeted communities, reduce racial and ethnic disparities and provide models for national reform.

AF4Q asks the people who get care, give care and pay for care to work together toward common, fundamental objectives to lead to better care.

Maine Aligning Forces for Quality (the Alliance) leads the area's AF4Q Maine initiative. For more information on the Alliance, visit their community snapshot or their web site.

The participation of consumer organizations in health care quality initiatives is critical to effect patient-centered changes in our health care system.

HealthWeb of Maine
HealthWeb of Maine provides a searchable database of state health data and statistics and links to other health-related information at the state and national level. This site was constructed to meet the needs of experts in the health care field as well as the public at large. HealthWeb of Maine is the product of a collaborative effort by the Maine Health Data Organization and the Phoenix Foundation of Portland, Maine.

The Leapfrog Group
The Leapfrog Group is a national public-private partnership that works to reduce medical errors and improve patient safety. The Leapfrog Group provides the latest hospital patient safety performance results in Maine.

Maine Health Management Coalition (MHMC)
MHMC is an employer-led partnership among multiple stakeholders working collaboratively to maximize improvement in the value of healthcare services delivered to MHMC members’ employees and dependents. The web site provides information on physician & hospital ratings, choosing a primary care & family care doctor, getting the right tests and check ups, and avoiding medical mistakes in the hospital.

Maine Medical Association
A website maintained by the Maine Medical Association, a voluntary association of over 2000 Maine physicians, provides links to free clinics throughout Maine.

Maine Quality Counts
Maine Quality Counts coordinates existing but disparate efforts across the state that support local, patient-centered, and coordinated systems of care and their allies. This is also the organization coordinating the Maine Medical Home Pilot.

Maine Quality Forum
The Maine Quality Forum's mission is to advocate for high quality healthcare and help each Maine citizen make informed healthcare choices. Here you will find information on how to choose a physician & hospital and data on variations of care, quality and disease burden.

National Committee for Quality Assurance (NCQA)
NCQA is a voluntary program designed to recognize physicians who use evidence-based measures and provide excellent care to their patients in the areas of heart/stroke care, diabetes care, back pain and systematic processes. Maine has physicians in all categories except back pain.

Northeast Health Care Quality Foundation (NHCQF)
NHCQF is the quality improvement organization for Maine, New Hampshire and Vermont. This organization works to ensure all Medicare patients receive high quality care.
