Aligning Forces for Quality/Humboldt County

The Americans for Quality Health Care Team works with the Aligning Forces for Quality (AF4Q) Alliances.

AF4Q is the Robert Wood Johnson Foundation’s (RWJF) signature effort to lift the overall quality of health care in targeted communities, reduce racial and ethnic disparities and provide models for national reform.

AF4Q asks the people who get care, give care and pay for care to work together toward common, fundamental objectives to lead to better care.

The Leapfrog Group
The Leapfrog Group is a national public-private partnership that works to reduce medical errors and improve patient safety. The Leapfrog group provides the latest hospital patient safety performance results in California.

California Department of Public Health— Health Disparities Information
This report has various statistics about health care disparities in California.

Health Services Advisory Group, Inc. (HSAG)
HSAG is California’s quality improvement organization. It works to protect Medicare patients rights, runs a Medicare beneficiary helpline, receives quality of care complaints, runs a discharge dispute program and works with patients on dispute resolution with their Medicare provider.

California Patient Safety Action Coalition (CAPSAC)
CAPSAC is a group of representatives from more than 20 of the state's leading public and private health care industry organizations who share the common goal of enhancing patient safety and increasing reporting of near misses and medical errors.
