Join us for a webinar about the Independence at Home (IAH) model, a model of care that uses interdisciplinary teams to bring coordinated primary care to older adults in their homes. During the discussion, we hope to identify the issues and opportunities of particular importance to patients and consumers and to use this as a platform for the Campaign’s advocacy in this area.
